Job Summary

To support development and operation of systems and processes within the Trust to ensure people with long term conditions are able to take or be administered medicines as prescribed in domiciliary settings.

Contribute operationally to the development and delivery of pharmaceutical, prescribing and medicines management initiatives across the PCT in accordance with national policy and the PCT medicines management strategy.

Assist in the delivery of rational, high quality, safe, cost-effective, evidence-based prescribing through integrated working.

Main responsibilities-Pharmacy Technician Job Description

1. Contribute to the delivery of prescribing and medicines management services across the PCT to meet the pharmaceutical care needs, improve concordance and reduce waste for patients.

 

2. Assessment of patient`s pharmaceutical needs to help promote concordance and follow up reviews to ensure that systems put in place are still effective.

 

3. Provide both written and verbal information to patients about their medicines.

 

4. Provide advice to carers on administration and handling of medicines as per the Medicines in Falls Support Programme Learning Pack

 

5. Liaise with Long Term Conditions teams, primary healthcare, secondary care, care agencies, social services, hospice, Age Concern to enhance care of the patient in domiciliary settings.

1. Build strong working relationships with GP practices, prescribers, community pharmacists and the multidisciplinary team to facilitate the delivery of the PCT Long Term Conditions Medication Review Service.

 

2. Communicate information accurately and clearly in order to realise changes in prescribing behaviour in accordance with national guidance e.g. NICE and local prescribing policy.

 

3. Communicate details of local pathways for referrals related to medicines compliance issues to all relevant stakeholders.

 

4. Communicate effectively with patients to assess their medication needs and explain the proposed action. Deal with any concerns they may have as a result of these actions.

 

5. Communicate with all relevant health and social care professionals on an individual patient basis to ensure best outcomes for the patient.

 

6. Work with other health and social care professionals to ensure effective liaison across all interfaces (secondary care, intermediate care, primary care, social care settings) with the aim of ensuring a seamless approach to medicines management.
