Job Purpose:

 

· To provide integrated social work services at an expert level including Protection of

Vulnerable Adults and mental capacity casework.

· Contribute to strategic direction of the integrated service.

· Provide management and leadership in professional and practice development to team members.

5. To represent CCS in multi-agency networks, working jointly with them in assessing, planning and implementing integrated care to promote independence and choice, admission avoidance and facilitation of discharge.

 

6. Regularly and consistently discharge responsibilities ordinarily falling outside the role of an experienced Social Worker or a Care Manager. Such responsibilities will include:

 

· Contributing to the development of knowledge and skills of other team members or students by sharing Knowledge and Practice

· Allocating and authorising care packages.

· Taking responsibility for professional development and training needs (e.g. practice teaching) including staff supervision.

· To induct new Care Managers and other staff as required.

7. To contribute to the development of integrated services and policy and to promote new ways of developing service user care, taking account of national initiatives and performance indicators.

8. To maintain and update knowledge regarding social policy and social work practice and be fully informed of PCT policy and procedures and relevant legislation, acting as a resource for the team and helping to develop others.

 

9. To take a key role in promoting and developing service user empowerment, recognising diversity and anti-discriminatory practice in all professional practice in the team, ensuring adherence to the National Occupational Standards for Social Work.

 

10. To take responsibility for own professional development.

11. Promote a positive and professional culture by assisting innovative practice and leading in a speciality interest area.

 

 

 

Qualifications and Training

 

· Qualified Social Worker with current GSCC registration and professional relevant qualification recognised for the

purposes of GSCC registration i.e. DipSW, CSS CQSW or Degree in Social

Work.

 

· PQI or Consolidation module or equivalent

 

· Willingness to undertake Practice Teacher/ Assessor award within 18 months

· Evidence of Continuous Professional

Development.

 

Experience-Senior Social Worker job description

· 4 years post qualification experience in Social Work practice.

ain Duties and Responsibilities-Senior Social Worker job description

 

1. To undertake highly complex case arrangements for assessment and care planning in situations which involve risk, complexity and safety of the service user and others.

 

2. To take a professional lead on adult protection issues within the Locality Team.

 

3. To work in partnership with service users and their carers to ensure the provision of a  service, including self directed support, which meets their assessed needs within available resources.

 

4. To provide more specialist social work practice skills and knowledge of Adult Services user group and to make independent decisions.

 

12. Take a lead role in the development of quality assurance systems within the team, ensuring the team achieve the Performance Indicators and Quality Assurance standards.

 

13. To undertake delegated management responsibilities, as determined by the Manager for Planned Care or Area Manager for Planned Care.

14. Provide professional leadership, teaching and mentoring.

 

Communication

1. Provide information and advice on services, charging policies and complaints process to service users and their carers, to enable them to make informed choices.

Human Resources

1. Participate in regular supervision, appraisal process and personal development reviews.

2. Assist in the identification, through Personal Development Reviews, staff potential and training requirements for the integrated team.

3. To complete post registration training and learning (PRTL) in accordance with GSCC requirements.

 

4. Provide professional leadership, teaching, mentoring, co-ordinating the Care Manager role and performance management by signing off cases on swift.

 

5. Assist in the recruitment process.

· Responsible for day-to-day supervision of staff (eg.work allocation and checking)

· Development of staff (Maintaining own professional development and requirement to take part in appraisal and KSF process)

 

Clinical and Practice Governance-Senior Social Worker job description

1. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

2. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to CCS Information Governance and Information Security policies.

5. It is a condition of your employment that you are currently registered with GSCC and it is your responsibility to maintain your professional registration.

8. Provide patients and their families /carers with information on standards they should expect from the team.

10. The post holder is required to participate in relevant emergency preparedness process for  their team.

 

 

General

The post holder mustbe aware of the responsibilities of all employees to maintain a safe and healthy environment for patients, visitors and staff.
